  1. Be 21 years of age
  2. Have consent of all adults living in your household
  3. Have a valid ID with your current address
  4. Provide your landlord’s name and telephone number (or a copy of your lease)
  5. Understand that PetEx Rescue ‘n Transport must approve your application.
